Please complete the form, attach copies of your invoices on the Liberty claim and mail the form (with the attachments) to:

You do not need to mail me copies since the clerk’s office will scan and electronically file the proof of claim and I will receive an electronic copy once the claim is filed.

Please do not include any original documents with the proof of claim since the clerk’s office will scan the claim and discard the documents.

For local creditors, you can also hand deliver the proof of claim to the clerk’s office.
